Does TRICARE Cover Marriage Counseling and Family Therapy in Addiction Treatment?

TRICARE covers family and couples therapy when it is medically necessary to treat a diagnosed condition, including substance use disorder. It does not cover marriage counseling on its own. So when family therapy is part of a loved one’s addiction treatment plan, it is covered. Relationship counseling with no diagnosis behind it is not.

When someone in a military family is in treatment for drinking or drugs, the family is in it too. A natural question follows: will TRICARE pay for the family sessions, and what about marriage counseling for the strain the addiction put on the relationship? The answer turns on one distinction, and once you see it, the rest falls into place.

Family therapy that is part of treating a diagnosed condition is covered. Marriage or couples counseling on its own, with no clinical diagnosis attached, is not. TRICARE states it plainly: it covers marriage counseling only when it is necessary for the treatment of a diagnosed mental disorder.1 Substance use disorder is a diagnosed condition, which is why family work tied to a loved one’s recovery falls inside the benefit.

What TRICARE covers, and the session limits

TRICARE covers outpatient psychotherapy when it is medically or psychologically necessary to treat a mental health disorder, and that includes therapy designed to treat the whole family. The covered session types and their time limits are set out on the official benefit page.1

There is no arbitrary cap on how many sessions you can have. The 2016 federal rule that modernized the benefit covers outpatient treatment that is medically or psychologically necessary, including psychotherapy and family therapy, without limits on the number of treatment sessions.2 The same rule recognizes certified marriage and family therapists as TRICARE providers, so the right clinician is covered.2

The line that trips people up

Here is where people get stuck. Marriage counseling on its own, the kind a couple seeks to work on the relationship with no diagnosis involved, is an explicit exclusion. TRICARE’s behavioral health guidance lists marital therapy among non-covered counseling services, alongside stress management and lifestyle counseling.3 So the same two people, in the same room, with the same therapist, can be covered or not depending on one thing: whether the therapy is treating a diagnosed condition.

In practice, the billing reflects that. When couples or family therapy is covered, one person is the identified patient with the diagnosis, and the therapy is part of that person’s treatment plan. If your loved one is in treatment for a substance use disorder, that diagnosis is already there, which is exactly why family sessions are routinely covered as part of addiction treatment under TRICARE.

Where family therapy fits in addiction treatment

Family work is not a separate, optional add-on. It is woven through the levels of care. In residential treatment, intensive outpatient, partial hospitalization, and standard outpatient counseling, the treatment team can bring the family in when it supports recovery. TRICARE covers substance use disorder treatment that is medically necessary across all of these settings.4

One useful detail for families who live far from a residential program: TRICARE generally does not cover telephone counseling, but it makes an exception for geographically distant family therapy related to residential treatment center care.3 So distance does not have to keep a family out of the work.

What about the cost?

Family therapy is billed like any other covered outpatient mental health visit, so what you pay depends on your plan, whether the provider is in network, and your enrollment category. There is no special surcharge for family or conjoint sessions. If TRICARE will not cover it, you still have options

If what you want is relationship counseling with no diagnosis behind it, TRICARE will not pay, but the military has free routes built for exactly this. Three are worth knowing:

Military OneSource offers free, confidential non-medical counseling to service members and their families, available by phone, video, or in person, with no cost and no referral. Call 1-800-342-9647, 24/7.5 Military and Family Life Counselors (MFLCs) are placed on or near installations and provide short-term, no-record counseling for couples and families. Chaplains offer confidential counseling and are often the most flexible option for relationship work. None of these requires a diagnosis.

The VA path, for veterans

If your loved one uses the VA rather than TRICARE, family work is part of VA substance use care too. The VA’s treatment menu includes counseling couples together on how to recover from substance abuse and improve relationships, and family members can take part in treatment planning when the veteran wants them involved.6 The VA also runs Vet Centers, community counseling centers that provide free counseling to combat veterans, service members, and their families, including for substance use.7 The Vet Center Call Center is 1-877-927-8387, 24/7. One more route for spouses: if a spouse does not qualify for TRICARE, CHAMPVA may apply, and it covers limited individual, family, and group therapy as part of substance use treatment.8

Doing this in San Antonio

Locally, both systems are within reach. The Audie L. Murphy Memorial Veterans Hospital, part of the South Texas Veterans Health Care System, runs the VA substance use program, and the San Antonio Vet Center offers family counseling close by.7 On the TRICARE side, families across Joint Base San Antonio can use on-base mental health resources or get a referral to an in-network family therapist. If a teen or child needs care, dependents listed in DEERS are covered for medically necessary mental health treatment the same way an adult beneficiary would be.1

The diagnosis is the key, not the room

The thing to hold onto is simple. TRICARE pays for therapy that treats a condition. When a substance use disorder is on the chart, the family work that supports recovery is part of treating it, and it is covered. When two people just want to work on the marriage, with nothing clinical behind it, that is when TRICARE steps back and Military OneSource, an MFLC, or a chaplain steps in.

Questions families ask

Does TRICARE cover marriage counseling?

Only when it is medically necessary to treat a diagnosed mental disorder. TRICARE does not cover marriage or couples counseling on its own. If the counseling is part of treating a diagnosed condition, such as a substance use disorder, it can be covered.1

Does TRICARE cover family therapy in addiction treatment?

Yes. Family or conjoint therapy is a covered service when it is part of a medically necessary treatment plan for a diagnosed condition like a substance use disorder. Regular family sessions are covered up to 90 minutes, and crisis sessions may extend up to 180 minutes.1

Will TRICARE cover couples therapy for me and my spouse?

Does TRICARE cover rehab or mental health treatment for a spouse?

Yes. Spouses enrolled in TRICARE are covered beneficiaries and can receive medically necessary substance use and mental health treatment, including inpatient, residential, and outpatient care, the same as the service member.4

Do I need a referral for family therapy or rehab on TRICARE Prime?

On TRICARE Prime, substance use disorder treatment at a rehab center generally needs a referral through your primary care manager. Routine outpatient mental health visits often do not require one. Check your specific plan, since rules differ between Prime and Select.

How many family therapy sessions does TRICARE cover?

There is no fixed cap. TRICARE covers medically necessary outpatient psychotherapy, including family therapy, without limits on the number of sessions, as long as the treatment remains necessary and is reviewed for appropriateness.2

What if we are not married yet?

TRICARE coverage runs through eligible beneficiaries, so a partner who is not yet a dependent is not covered as a couple. Free options that do not require marriage or a diagnosis include Military OneSource counseling, MFLCs near most installations, and chaplains.5

Does TRICARE cover therapy for a dependent teen or child?

Yes. A child or teen listed as a dependent in DEERS is covered for medically necessary mental health and substance use treatment, including therapy. On Prime, care usually starts with a referral from the primary care manager.1

Can I see a family therapist off base, and any one that takes TRICARE?

You can see any TRICARE-authorized provider. On Prime you typically need a referral first, then TRICARE helps assign an in-network provider. On Select you can choose your own in-network therapist. Confirm the provider is currently accepting TRICARE before booking.

What is a collateral visit, and is it covered?

A collateral visit is a session between the provider and a family member or other responsible person, rather than the patient. TRICARE covers collateral visits when they are needed as part of the patient’s care, which is useful when a spouse or parent needs to be part of the treatment without being the patient.1
